##  Summary 

 



Use this worksheet form to complete a construction confined space entry permit before entering a [permit-required confined space](/resource/confined-spaces "Confined Spaces") on a construction project.

- For Harvard-identified confined spaces, contact EHS to request a confined space evaluation before completing your permit.
- The entrant, attendant, and entry supervisor authorize the permit by signing the form before entry.
- Permits are valid for eight hours or until the job is complete (whichever comes first).
