This form is for labs in Cambridge buildings. If your lab is in a Harvard Longwood Campus building, contact your [Lab Safety Advisor (LSA)](/resource/ehs-safety-officers "EHS Safety Officers") to apply for a controlled substances license.

Before submitting a controlled substances license application to federal and state regulatory agencies, the Principal Investigator (PI) must:

1. Complete this form.
2. Forward the form to the department administrator.

The PI must update this form as needed.
