This form is for labs in Cambridge buildings. If your lab is in a Harvard Longwood Campus building, your Principal Investigator (PI) can purchase controlled substances directly.

Before purchasing controlled substances, the PI or their authorized individual must:

1. Complete this form.
2. Submit the form to the department administrator.
