**Summary**

You must authorize a Harvard hot work permit form for each hot work activity.

1. Complete the permit form.
2. The permit-authorizing individual (building manager), hot work operator, and hot work fire watch sign the form.
3. Post the permit form onsite during hot work activities.

Each permit is authorized for one work shift.
