Chemical Hygiene Plan and Exposure Control Plan Annual Review Form

Use this form to document annual reviews of your lab’s chemical hygiene plan (CHP) and exposure control plan (ECP):

1. Record the name of the Principal Investigator (PI).
2. Review your CHP (Part III Site-Specific Procedures) and ECP and update each plan if necessary.
3. Record the reviewer’s signature, applicable plans, and date of review or update.
4. Keep the completed form with your CHP.
