CSAP/SAPP Enrollment Portal


You need to enroll your construction project in the Contractor Safety Assessment Program (CSAP) and the Substance Abuse Prevention Program (SAPP) if your project value exceeds $100,000. Construction projects include renovations, new construction, tenant fit-outs, utility upgrades, major repairs, etc.

Before the project begins, fill in both the CSAP Project Enrollment form and the SAPP Screen Passport Request form with your project information.

CSAP Project Enrollment

The CSAP, operated by a third party, allows the University to assess its contracting partners' safety performance and assist the project management community in making informed decisions.

Visit the CSAP Project Enrollment form to enroll in the CSAP program.


Make sure you have the following information for the form:

  • Project Name
  • Harvard Project Manager
  • Harvard School
  • Anticipated Start and Completion Date
  • Contractor Name and Address
  • Approximate Contract Value
  • Building Use and Construction Type

After the form is submitted, both the Harvard Project Manager and Contractor Project Manager will have access to the construction project's safety information.

SAPP Screen Passport Request

Harvard University requires pre-hire substance abuse screening of all construction workers by a third-party administrator. Before getting tested, projects must request a SAPP test passport.

Visit the SAPP Screen Passport Request form to request a drug testing passport.


Make sure you have the following information for the form:

  • Project Name
  • Harvard Project Manager
  • Harvard School
  • Contractor Name and Address
  • Designated Employer Representatives (DER)

Allow up to 24 hours to receive the SAPP testing passport.

The DER will receive an emailed copy of the SAPP testing passport within 24 hours. The DER must sign the passport. After the passport is signed, the DER sends it to all workers working on the jobsite. Workers must receive the passport before SAPP testing.

Important Information

Workers must bring a picture ID and signed paper copy or a signed digital copy of the passport to the testing facility.
The employee will not be allowed to test without the passport.

If your DER changes during the project, notify OHS Total Care.

After passing the SAPP test, each worker will receive a small business sized plastic card known as a "P Card.” They will need to keep this with them while on Harvard University projects.
